Louisiana 4-H Overnight Event/Health and Code of Conduct Forms and 4-H Enrollment Cards
To participate in various 4-H events, 4-H'ers must complete a Louisiana 4-H Overnight Event Permission/Health Form and Code of Conduct Form. If a 4-H'er takes prescription medication(s), the Medication Consent Form also must be completed. These forms must be signed by the 4-H member's parent.
